Clustered ring internal NME enhancement indicates a cluster of small ring-shaped enhancing foci within the NME region — ring-shaped enhancement that is grouped together.
Definition
A cluster of small ring-shaped (hollow ring) enhancing foci within the NME. Each focus has a ring of enhancement with a relatively dark center, and these ring foci are grouped together.
Imaging Appearance
- Early post-contrast T1W: multiple small ring-shaped enhancing foci clustered together
- Individual rings are hollow (enhanced periphery, dark center)
- Grouped in an NME distribution pattern (segmental, diffuse, etc.)
Pathological Basis
Clustered ring enhancement corresponds to small neoplastic foci with central necrosis or to tumor cells arranged around central lumens — classically associated with high-grade DCIS with comedonecrosis.
Suspicion Level
Suspicious — Clustered ring enhancement is a concerning internal NME pattern.
Board Pearl
Clustered ring enhancement in NME is suspicious and strongly associated with DCIS (particularly high-grade or comedo-type). This pattern in a segmental or diffuse distribution warrants biopsy.
